A government mistake has reportedly led to the loss of a British woman’s life. Tamara Jade Logan, a mom and makeup artist living in Glossop, Derbyshire, took her life after being told that her government benefits were being cut. Logan, who had a history of mental health symptoms that included suicidal ideation, found the news unbearable. It’s unknown how much her mental health affected her ability to work — not many details about Logan have been released. We do know, however, that after she was told about her loss of benefits, an inquest after her death determined the news was incorrect. Sadly, it was too late.
The 28-year-old was informed by the Department of Work and Pensions that she would no longer be receiving the “enhanced daily living allowance” portion of her Personal Independence Payment, the Daily Mail reported.
Senior south Manchester coroner Alison Mutch wrote in a prevention of future deaths report that when Logan received the letter, her “mental health deteriorated further.”
“The decision to remove the enhanced payment has been accepted as an incorrect determination. The method used for communication of the decision was also not appropriate given her known vulnerabilities,” Mutch shared, per the Daily Mail.
Logan was found hanged on May 18, 2025. She was taken to Tameside General Hospital, where she died two days later.
According to Mutch, “the method of communication of the decision significantly contributed to her declining mental health and her actions on 18th May 2025,” the outlet noted.

Logan was a “vulnerable person with a history of self-harm and suicidal ideation,” Mutch shared, according to the Manchester Evening News. The Department of Work and Pensions was aware of her mental health; her file noted previous self-harm and suicidal ideation.
“It was clear from the evidence that her vulnerabilities were recognised by the Department of Work and Pensions and their paperwork was flagged to that effect. Despite that a standard letter was sent with no attempt to reduce the risk that receipt of the letter would cause,” Mutch wrote, per the Evening News.
The coroner determined Logan died by suicide, with the medical cause of death as “hypoxic brain injury” caused by “hanging.” The report was sent to the Secretary of State for Work and Pensions, which has until March 19, 2026, to respond.
A spokesperson from the department of Work and Pensions shared a statement on the death of Tamara Jade Logan. “Our sincere condolences go to Ms Logan’s family and friends. Protecting the millions of people we support every year is a priority. We take the Coroner’s comments extremely seriously and will provide a full and detailed response.”
